Specialty Care Phase II Implementation in action
SCOPE: Recently a multi-home organization asked Specialty Care to provide Phase II Implementation across all of their homes to:
- improve resident care,
- fully implement clinical modules to improve documentation and auditing
- reduce duplication and staff documentation time.
They chose Specialty Care because of our expertise and partnership with PointClickCare and over 25 years LTC experience. Specialty Care provided on site training for all clinical aspects including assessment, care plan, progress notes, weight monitoring, flow sheets, quarterlies, shift reports, audit reports and CMI maximization. We also provided proficiency checklists and 'super user' training for each home along with policies and assessment form templates and a Tips Binder with some one-on-one support.
RESULTS: The immediate result was a more consistent approach to clinical documentation including use of common assessment tools, more consistent and complete assessments and by sharing tips between co-workers as their skills and confidence increased. This has led to better auditing practices comparing similar issues with more confidence knowing everyone was inputting the same way and with more content. Documentation became more efficient as duplication was reduced. The skills and knowledge level of staff members have improved through Specialty Care Phase II training, positioning them well for future implementation of MDS.
